Please read the following instructions carefully before submitting your certificate details.

1. Only students who have successfully completed the course and passed the final examination are eligible to register their certificate.
2. All fields in this form are mandatory. Incomplete or incorrect information may result in certificate rejection.
3. Please enter your Full Name exactly as it appears on your certificate.
4. Ensure that the Certificate ID is entered correctly (Example: KYASC-123456).
5. The percentage must be entered accurately as shown in your exam result.
6. The mobile number provided should be active for future communication.
7. After submitting this form, your certificate details will be recorded in the official database of KYASC Institute.
8. Certificate verification can be done using the Certificate ID on the verification page.
9. Providing false information may lead to permanent cancellation of certificate validity.
10. Once submitted, the details cannot be modified. Please review all information before final submission.

Note: Digital signature is integrated in the certificate. Physical signature is not required.
